QUICK START

• Check that the microscope camera is transmitting signal
• Make sure the HDMI cable from the camera is connected to the capture card
• Confirm that the capture card is properly connected via USB to the mini PC

• Verify that the smartphone and the mini PC are connected to the same Wi-Fi network
• Ensure the network is stable
• Confirm that the IP configured in the app and in the mini PC software matches
• Press the circle in the bottom-left corner to see server information

• Make sure the purple limbal mark is correctly detected
• Avoid fluid, viscoelastic, or bleeding, as these can interfere with detection
• Ensure the eye is centered and the entire limbus is visible in the image
• Reduce excessive light reflections from the microscope camera

The main screen of the ToricAR software on the mini PC includes all control and monitoring functions before and after surgery.

Main elements

• Language selector: appears at first startup and can be changed at any time
• Connection status: circle in the lower left corner
  • Green: correct connection, ready to receive data
  • Red: the system is offline
  • Clicking the circle displays server details, data reception status, and tips to improve the connection
• Video preview: shows the live feed from the microscope camera
  • With a double click on the video, it opens in full-screen passthrough mode, i.e., direct video only with no processing or detection
• Data reception indicator: below the video, the data sent from the ToricAR app is displayed
  • Patient name
  • Eye to operate
  • Selected surgical axis
  • Incision configuration
  • Rexis size
• Start Surgery button: opens the surgical screen with iris detection and overlay guides
• "i" (information) button: located in the lower right corner
  • Displays the current IP and port of the server
  • Provides access to the Recordings menu, where all videos created within the software are listed
  • From this menu, it is possible to view and export the recordings created in ToricAR to a USB drive

The surgery screen is where the ToricAR system enters active assistance mode, with real-time detection and surgical overlays.

Main elements

• Full-screen video: shows the live feed from the microscope
• Automatic iris detection: represented by a blue circle over the patient's iris
• Surgical mark detection: when present, displayed as a yellow dot on the limbus
• Patient data and surgical configuration: shown in the top-right corner of the screen (name, eye, selected axis, incisions, and rexis)

Main controls

• Fixate iris:
  • Should only be pressed when both the iris circle and yellow mark are correctly detected
  • Once activated, it stabilizes the reference and compensates for cyclorotation
  • With the iris fixated, you can use the mouse scroll to adjust the size of the detected iris circle
• Show axis: overlays a dashed line on the programmed axis for IOL alignment
• Show incisions: displays the incisions configured in the ToricAR app
• Show rexis: overlays the configured size and position of the rexis

Recording

• Pressing the ToricAR logo on the surgery screen starts or stops video recording
• Recorded videos are saved in the Recordings folder and can be accessed from the recordings menu on the main screen

Finalization

• Finish button: closes the surgery screen and returns to the main screen

Patient information and eye selection

• The patient's name is shown at the top of the screen
• When selecting the eye to operate, it is highlighted in the interface
• If an eye already has saved data, a check mark appears over it

Progressive workflow of the app

• Each configuration button becomes active progressively, as the previous steps are completed
• The data sending slider is only enabled once the entire configuration has been completed

Photo capture

• At the start, the app guides the user to take a close-up photo of the eye with the surgical mark on the limbus
• The camera helps keep the device in a vertical position, or automatically compensates orientation to align the horizontal meridian

Iris and mark adjustment

• After taking the photo, the app detects the iris and highlights it with a circle
• The user can manually adjust the circle by dragging it with a finger or resizing it with a pinch gesture
• Detection of the purple limbal mark is semi-automated; if necessary, the user can manually place it at the exact position

Surgical configuration

• After adjusting the photo, the surgical setup screen is displayed:
• Implant axis: selected with a circular slider and fine adjustment buttons (±1°)
• Incisions: position and size can be configured
• Rexis: requires the WTW (White-to-White) value to activate this function

"i" (information and network) button

• Pressing the "i" button opens a menu with additional functions:
• IP and port configuration for communication with the mini PC
• Reference data: shows the parameters extracted from the captured photo (e.g., iris center and radius, mark position, etc.)

The ToricAR system can be complemented with a surgical foot pedal (not included in the basic setup). This accessory allows hands-free control of key functions during surgery.

• Fixate iris: stabilizes the iris reference, compensating for cyclorotation
• Show guides: toggles visualization of the axis, incisions, and rexis during surgery
• Record surgery: starts or stops recording of the surgical screen
